In Oregon, a contract cleaner was killed when cleaning a meat processing facility and fell into a meat grinder. Avalos-Chanon, 41, died from “blunt-force injuries and chopping wounds.”
An OSHA inspection last Fall found that machines were not locked during the tear-down process for cleaning.
